To ensure prompt processing of your claim, it is important that
you submit a completed Claim Form and the Original Policy/Certificate
of Insurance together with the applicable documents to:

IMPORTANT NOTICE

- Employers must report All Work-related Accidents
involving their workmen as soon as possible to Ministry
of Manpower (MOM) - Work Injury Compensation Department
and to AHA
- Deadlines for reporting accidents to MOM :
Where the accident
results in death
> Within 10 days of the occurrence |
Where the accident
renders the workman unfit for work for more than 14
days
> Within 18 days of the occurrence |
Where the accident
renders the workman unfit for work for 14 days or
less
> Not later than the 15th day of the
month following its occurrence. |
- Failure to report a work-related accident to MOM is
an offence punishable with a fine of up to S$2,000.00.
- Submit original medical bills, receipts, medical certificate
and all other correspondence to & from MOM
